Kaibab Lake Team Building October 1, 2024

Ash Fork Middle School will be going to Kaibab lake in Williams on Tuesday, October 1, 2024. We will be playing an evade and capture type game in order to win advantages for further team building games at Kaibab Lake.

When: October 1, 2024

Itinerary: Leave school in the morning after taking attendance. Go to Kaibab Lake and compete in various teambuilding activities around Kaibab campground before and after lunch. We will not be getting wet. If time permits, we will be taking a short hike on an established trail through the woods near the Williams Forest Service station.

Clothes: Your child will need to wear clothes for being outside in the woods (pants, long sleeves, or short sleeves with a jacket or sweatshirt, hat). Your child needs to wear shoes that are appropriate for running over rocks and grass. No Crocs!

Other essential items: Ensure your child brings a backpack and water bottle.

Food: The school will be providing a sack lunch for your child.

Contact: If you have any questions, contact Mrs. Broehm, Ms. Acosta, or Mr. Staples

Ash Fork Joint Unified School District #31 Participation in the Kaibab Lake and Williams Hiking Team-Building Consent Form

Student Name: _________________________________________________________

Parent/Legal Guardian Name: _____________________________________________

As the parent or legal guardian of the above-named student, I give permission for my child to participate in the Kaibab Lake and Williams Hiking Team Building Trip (the Trip), which will take place on October 1, 2024.

Participation in this Trip involves hiking in the outdoors that may involve risk of serious injury, including falling, permanent disability, and death and damage to personal property, which might result not only from my child’s own actions, inactions or negligence, but from the actions, inactions or negligence of others or the conditions of the premises or of any equipment used. This trip includes playing games and hiking through the woods so my child will be exposed to various risks typically associated with hiking and playing in the outdoors. Such risks include uneven terrain, adverse weather conditions, exposure to insects, reptiles, and plants. Knowing the risks, I give my child permission to participate in these activities.

I give permission for Ash Fork School District employees or volunteers to seek medical treatment for my child as necessary in their judgment. I am aware that Ash Fork School District does not provide accident or health insurance coverage for my child and have independently determined whether I should obtain such insurance at my own cost. I understand that the student’s accident or health insurance or the parent of the student will pay for any health care and expenses incurred. Payment of such expenses is not the school district’s responsibility.

I understand that my child’s participation is conditional on maintaining proper and respectful behavior towards the school district staff, volunteers, and other students. The same code of conduct required of my child while he or she is in school applies to this Trip. If my child misbehaves, then he or she may lose the privilege to participate in the activities of this Trip. I have read and understand the foregoing information and acknowledgments. I understand that this agreement is legally binding between the parties outlined above and affects any legal right to claims for damages in the event of injury, illness, damages or death. My agreement to my child’s participation is voluntary and without duress.
