Dear Parents,

It's hard to believe that school is already halfway done, and we only have one middle school team-building activity left!

On Tuesday, January 9, 2024, the Ash Fork Middle School will visit Yavapai College to learn about the Mountain Institute Career Technical Education District (MICTED) classes. Yavapai College works with high schools from Yavapai County, offering over 20 different career and technical education classes that high school students can take. Ash Fork Middle School encourages students to consider MICTED courses because they provide unique opportunities. Completing these classes gives students a certificate that allows them to enter a career upon leaving high school. For example, a student who earns a welding certificate can earn, on average, between $41,610,00 and $69,090 after high school. Click on the link below for more information about the career and technical education classes offered through Yavapai College.

After learning more about the MICTED classes, team-building will move to the Constellation Trail and Watson Lake in Prescott, Arizona. The Constellation Trail has an outer loop that spans 3.53 miles. The trail is named after an airplane that crashed in that location. Hikers can see a memorial and wreckage of the plane on the trail. Watson Lake is a man-made lake created in 1915 by damming Granite Creek. The lake is a great place to fish and has a large grass field that our students will use for team-building games.

Permission slips will be coming home on Monday, January 8, 2024. However, you can view the permission slip below.

 Student Name: _________________________________________________________

Parent/Legal Guardian Name: _____________________________________________

As the parent or legal guardian of the above-named student, I give permission for my child to participate in the Constellation Trail-Watson Lake Team Building Trip (the Trip), which will take place on January 9, 2024.

Participation in this Trip involves hiking and playing in the outdoors that may involve risk of serious injury, including falling, permanent disability, and death and damage to personal property, which might result not only from my child’s own actions, inactions or negligence, but from the actions, inactions or negligence of others or the conditions of the premises or of any equipment used. This trip includes playing games and hiking along the Constellation Trail and at the Watson Lake grass field so my child will be exposed to various risks typically associated with hiking and playing in the outdoors. Such risks include uneven terrain, adverse weather conditions, exposure to insects, reptiles, and plants. Knowing the risks, I give my child permission to participate in these activities.

I give permission for Ash Fork School District employees or volunteers to seek medical treatment for my child as necessary in their judgment. I am aware that Ash Fork School District does not provide accident or health insurance coverage for my child and have independently determined whether I should obtain such insurance at my own cost. I understand that the student’s accident or health insurance or the parent of the student will pay for any health care and expenses incurred. Payment of such expenses is not the school district’s responsibility.

I understand that my child’s participation is conditional on maintaining proper and respectful behavior towards the school district staff, volunteers, and other students. The same code of conduct required of my child while he or she is in school applies to this Trip. If my child misbehaves, then he or she may lose the privilege to participate in the activities of this Trip. I have read and understand the foregoing information and acknowledgments. I understand that this agreement is legally binding between the parties outlined above and affects any legal right to claims for damages in the event of injury, illness, damages or death. My agreement to my child’s participation is voluntary and without duress.
